Prior to your appointment time with Matt, we require that you complete the “Preliminary Information” form as your homework. You can download the form below by selecting the version that best describes your status.
Simply select the form that best pertains to you, and fill it out as completely as possible. Once completed, you may fax it, mail it, or contact our office for the appropriate address to send it via email. Matt has the heart of a teacher but he doesn’t accept late homework! You have one week to complete the assignment and turn it in for his review.
(Please note: Depending upon the internet browser you utilize, the forms may not be fillable for you. They work optimally on Internet Explorer with the latest version of Adobe Reader. If you are not able to use the fillable form, simply print it out and fill it in, or contact our office for a Word version of the form.)
