Prior to your appointment time with Matt, we require that you complete the “Preliminary Information” form as your homework. You can download the form below by selecting the version that best describes your status.
Simply select the form that best pertains to you, and fill it out as completely as possible. Once completed, you may fax it, mail it, or contact our office for the appropriate address to send it via email. Please call the office if you have any questions as you complete your form.
